Responsibilities

If you’re someone who believes that coordinated support can make a real difference in people’s lives, this role was built for you. As an Integrated Care Extender at Health Connect America, you’ll work alongside Integrated Care Coordinators to help clients access the services, resources, and guidance they need to thrive. Your contributions will be hands-on, relationship-driven, and essential to promoting continuity of care and improving client outcomes.

What You'll Do

As an Integrated Care Extender, you’ll assist care coordinators in delivering person-centered support and ensuring clients stay connected to the care and resources they need. Day to day, you can expect to:

  • Conduct outreach, engagement, and follow-up with members to ensure ongoing connection and support

  • Coordinate services and appointments, including wellness reminders and transportation assistance

  • Engage in health promotion and education activities, sharing knowledge and resources to support client well-being

  • Communicate member circumstances and updates to care managers and the broader care team to inform decision-making

  • Track and manage referrals, providing guidance and assistance in accessing community resources and social supports

  • Participate in case conferences and team meetings to support collaborative care planning

  • Assist care managers in assessing and addressing unmet health-related resource needs

  • Maintain timely, accurate documentation of all activities within the care management platform (ONEcare)

What to Expect

This role combines client engagement, administrative support, and collaboration with a multidisciplinary team. You’ll work closely with care managers, clients, and community partners to ensure that care coordination activities are effective and client-centered.

You’ll also participate in virtual team meetings and attend in-person trainings or meetings as needed, supporting professional development and program quality.

Supporting Quality & Compliance

Your work will help ensure that Health Connect America’s integrated care services meet organizational standards and regulatory requirements. Through accurate documentation, active engagement, and consistent follow-up, you’ll play a vital role in delivering high-quality, coordinated care.

Qualifications

We welcome candidates who bring a genuine commitment to supporting individuals with developmental or behavioral health needs and meet the following qualifications:

  • Must be at least 18 years of age

  • Minimum of a high school diploma or equivalent credential

  • Must meet one of the following:

    • A person with lived experience with an I/DD or TBI, with demonstrated knowledge of and direct experience navigating the North Carolina Medicaid delivery system

    • A person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist

    • A parent or guardian of an individual with an I/DD, TBI, or behavioral health condition, with at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their own family member)

  • Minimum of two years of paid experience in roles similar to the Extender Functions, with at least one year of hands-on experience working with the Tailored Care Management eligible population
