As an Integrated Care Coordinator at Health Connect America, you’ll serve as a key connector for clients, providers, and community resources—ensuring comprehensive behavioral and physical health support, closing gaps in care, and promoting overall well-being. Your work will be hands-on, relationship-driven, and integral to both client outcomes and program success.
As an Integrated Care Coordinator, you’ll oversee care planning, coordination, and monitoring for assigned clients, supporting both behavioral and physical health goals. Day to day, you can expect to:
Conduct assessments and develop individualized, person-centered care plans based on client needs, health goals, and Social Determinants of Health (SDOH)
Coordinate and monitor services across treatment teams, primary care providers, and community resources to ensure seamless care delivery
Facilitate follow-up services for recent hospitalizations, life transitions, or other critical events to support smooth transitions of care
Monitor client progress, track HEDIS gaps, and verify enrollment and payer status to ensure access to all required services
Provide health education, guidance, and crisis response support to clients and their families as needed
Maintain timely, accurate documentation of care coordination, assessments, and interactions in accordance with regulatory and agency standards
Support clinic-based operations, including scheduling, reminders, vitals, chart documentation, and direct collaboration with Nurse Practitioners for integrated care clients
Participate in marketing initiatives, outreach to new referrals, and education of prospective clients to promote engagement and program enrollment
This role requires strong organizational skills, clinical insight, and the ability to navigate complex care needs across multiple providers and settings. You’ll balance in-office responsibilities with community and client-focused engagement, supporting both ongoing wellness and crisis intervention as needed.
As part of your role, you’ll collaborate closely with treatment teams, integrated care staff, and community partners, ensuring that every client experiences comprehensive, coordinated care that addresses both immediate needs and long-term health goals.
You will ensure all care coordination activities comply with Health Connect America policies, state regulations, and program standards. Through diligent documentation, adherence to timelines, and proactive monitoring, you’ll help maintain the highest quality of service while promoting positive client outcomes.
